How to document wounds

For the OASIS SOC, every wound must be entered from scratch. For revisit notes, the wounds widget lets you see patient’s existing wounds that are already in WellSky.

Use the app to add pictures to the wound manager and document your wound care for this visit. You can also discontinue wounds that are fully healed.

What you must complete in Kinnser

Andy completes your assessment, narrative, and updates the wounds widget in 1 to 8 hours. During your review, do the following:

1. Review any yellow sticky notes for missing info or abnormal values to notify MD.
2. Add meds to the med profile by hand (AI is coming soon!)
3. Check off any interventions performed or goals met in progress-to-goals
