3 ways to ask Andy to document your meds:

✅ Take a picture of the handwritten med list

✅ Take a picture of the discharge paperwork

✅ Take pictures of pill bottles

❌ Do not say the meds out loud

Option 1: Paper med list

If you currently leave a handwritten med list in the patient’s home, the easiest way is to take a picture of the med list.

✅ GOOD: Meds should be legible by the patient

❌ BAD: These meds are unclear / missing information

Option 2: Discharge paperwork

If you print out discharge paperwork, that can work too! Just make sure to cross off old meds and write-in new meds.
