Tell Andy as if you were telling a fellow clinician. Andy will take into account your recordings and the patient’s past medical history. You can do this during the visit, or during the debrief.

Patient mostly sits in her wheelchair. She’s technically able to walk but she is afraid to lose her balance and fall again, so she doesn’t really walk.

For therapists

You can use common therapy phrases such as min assist or max assist.

For OASIS visits: Andy will ensure that the therapy eval matches the M1800s and GGs.

For routine visits: Andy will only document specific assessment items, and leave the other ones alone. For example, if you say “patient is min assist for car transfer” then we Andy will document car transfer, and leave all of the other levels alone.

What not to say

❌ M1800 is 2, M1810 is 3, etc…
