Tell Andy as if you were telling a fellow clinician. Andy will take into account your recordings and the patient’s past medical history. You can do this during the visit, or during the debrief.

✅ Patient ambulates with a walker inside the home, and uses a wheelchair for longer distances

✅ Patient can’t go up the stairs. Patient needs a walker, but prefers to steady himself using furniture

✅ Patient uses a wheelchair, mod assist for transfers

✅ Patient is wheelchair bound, has a hoyer lift for transfers

For therapists

You can use common therapy phrases such as min assist or max assist.

What not to say

❌ M1800 is 2, M1810 is 3, etc…
